Social Security Consultative Examinations
These evaluations are scheduled by Social Security Disability Determination Services. We evaluate all ages, from children through adults. If you have questions about an upcoming or past evaluation in our office, we will try to help, but the best contact would be your assigned disability evaluation analyst. Their name and number will be on your appointment confirmation letter. Prior to your appointment you will receive a reminder call where you can confirm your appointment. If you have any scheduling difficulties such as wanting to arrive before or after your scheduled appointment time on the same day, please let them know during that call and they will do their best to accommodate you. If you are unable to attend your scheduled appointment, please let them know and then contact your assigned disability evaluation analyst as they are the only one who can reschedule your appointment. Please arrive at least 15 minutes before your exam time and bring a valid government ID and if you are currently taking psychiatric medications, bring a current list or even take pictures with your phone (makes it easier than bringing all your meds in a bag).
